李白《长干行》里“郎骑竹马来,绕床弄青梅”，本来就是孩童们的游戏，青梅竹马的意思就是从小玩到大的一对男女。翻译起来的基本意思就是楼上的。

Puppy love 算是英语中比较对应的说法了。
当然你定要文绉绉地体现这份纯洁的情谊，可以用childhood sweetheart来表示；或 childhood playmate（儿时玩伴，这个不分性别）也可以。

青梅竹马（只有最后一个解释勉强凑和，中文对应的意境是翻不出来的）：
Childhood friends//
a friendship formed in childhood--- said of boys & girls//

阿甘正传里面讲到 Jenny and I like peas and carrots，peas and carrots 指形影不离，不过我觉得用来形容两个小孩子从小很要好的状态也很合适，因为豌豆和胡罗卜丁在配菜中，一个是绿的，一个是红的，一个是圆的，一个是方的，各不相同但相得益彰，和“青梅竹马”异曲同工:)
